An issued outpatient card about a conducted ophthalmologic examination, or an examination slip issued by an optometry shop, containing а prescribed vision correction and stickers for purchased diopter/contact lenses - a copy
A prescription, issued by the attending physician, for purchasing dioptric lenses or contact lenses – a copy
A detailed invoice in the Insuree’s name (reflecting the work done to position lenses in eyeware frames, frame and glasses) and a fiscal receipt, attached to it - an original
An official document, containing the Insuree’s bank account, if it has not been indicated in the Claim Payout Form
