Upon Insuree’s permanent disability due to a general illness or an occupational disease, the following documents should be submitted:

1. Claim Request Form Л3-53 – an original with all particulars, as all documents should be listed in the form
2. Copy of the identity card of the eligible person for Life insurance and Group Risk Life insurance
3. Medical documents, pertaining to the illness: discharge summaries, outpatient cards, Personal Medical Record (PMR), medical referrals and interpretation of imaging test results
4. FIRST decision of the Disability Assessment Committee/ National Disability Assessment Committee – a copy
5. An official document, containing the bank account of each and every heir, if it is not indicated in the Claim Request Form. For amounts over BGN 10 000, the submission of an official document certifying the IBAN number of a bank account is mandatory.
6. Upon group insurance: Company Certificate from the Employer that the person is insured - an original

In case the sums will not be received by the eligible person:

Notarized power of attorney from the entitled person, if the sums will be received by another person. The power of attorney must contain a statement (text) that the eligible person has been notified that he is entitled to receive the sums in person.
