2. Upon Life Insurance policies with a savings component: Declaration Statement under Article 142t, Para 1 of the Tax and Social Security Procedure Code (TSSPC)
3. A transcript of a Death Certificate – a copy
4. Death Notice - a copy
5. ACCIDENT Information Form ЛЗ 51 – an original
6. Legal Heirs Certificate, if those are eligible – a copy
7. An official document, containing heirs’/beneficiary’s bank account, if it is not indicated in Form ЛЗ 53 – a copy. For amounts over BGN 10 000, the submission of an official document certifying the IBAN number of a bank account is mandatory.
8. Copy of the identity card of the eligible person for Life insurance and Group Risk Life insurance
9. Medical documents for rendered first aid and treatment conducted - discharge summary, outpatient cards, test results, etc. - copies
10. Forensic report when required by the investigators
11. Documents, issued by the investigative and judicial authorities, if a lawsuit has been initiated
12. Upon a road accident*– a document from the control authorities that have visited the accident scene
13. Upon a road accident* – a document for a taken blood sample for alcohol testing and its results, if the injured person is a vehicle driver
14. Upon individual and family insurance: Insurance policy
15. Upon group insurance at the Employer’s expense: Company Certificate from the Employer that the person is insured
16. Upon group insurance at the Insurees’ expense: A copy of the insurance policy and the list of Insurees’ names

Upon death due to an accident at work, the beneficiary should provide the following additional documents::

1. Declaration Statement to the National Social Security Institute about an accident at work - a copy
2. NSSI order about an acknowledged accident at work - a copy

In case the sums will not be received by the eligible person:

Notarized power of attorney from the entitled person, if the sums will be received by another person. The power of attorney must contain a statement (text) that the eligible person has been notified that he is entitled to receive the sums in person.
