Upon the Insuree’s death due to a general illness or occupational disease, the beneficiaries should provide the following documents:
- Claim Request Form Л3-53 – an original with all particulars
- Upon Life Insurance policies with a savings component: A Declaration Statement under Article 142t, Para 1 of the Tax and Social Security Procedure Code (TSSPC)
- A transcript of a Death Certificate – a copy
- Death Notice – a copy
- All medical documents, associated with registration and treatment of the illness that caused the Insuree’s death – outpatient cards, tests, discharge summaries, history of the illness, autopsy report, etc.
- Legal Heirs’ Certificate when those are indicated as eligible – a copy
- An official document, containing the bank account of each and every heir, if it is not indicated in the Claim Request Form. For amounts over BGN 10 000, the submission of an official document certifying the IBAN number of a bank account is mandatory.
- Copy of the identity card of the eligible person for Life insurance and Group Risk Life insurance
- Upon individual and family insurance: Insurance Policy - the original or a declaration statement on a lost insurance policy
- Upon group insurance at the Employer’s expense - Company certificate from the Employer that the person has been insured as at the date of the event – an original
- Upon group insurance at the Insurees’ expense – a copy of the insurance policy and a list of Insurees’ names
In case the sums will not be received by the eligible person:
Notarized power of attorney from the entitled person, if the sums will be received by another person. The power of attorney must contain a statement (text) that the eligible person has been notified that he is entitled to receive the sums in person.
