In case of purchased consumables for surgical treatment - the invoice should contain consumables’ description by type, number and unit price, stickers of consumables used, while the discharge summary should contain an excerpt of the surgery report, listing all consumables used) - a copy
An official document, containing the Insuree’s bank account, if it has not been indicated in the Claim Payout Form
